This is a copy of the paperwork a customer is expected to sign before Frank's Collision Repair can begin repairs to your vehicle:

I herby authorize Frank's Collision Repair to perform the necessary repairs and to obtain the necessary parts/materials to repair my vehicle per the insurance company estimate or shop estimate.  I understand that Frank's Collision Repair will obtain approval from the paying insurance company or myself for any additional repair costs that may be required once the repairs have begun.  Hereby, I grant Frank's Collision Repair and its employee's permission to operate my vehicle on streets, highways, or elsewhere for the purpose of testing, inspection, and/or sublet repairs.  An express mechanics lien is hereby acknowledged on my vehicle to secure the amount of repairs thereto.  It is understood that this company and its employees are not responsible for any lss or damage to the vehicle or articles left in the vehicle in case of fire, theft, or any other cause beyond their control.  I understand that vehicles that are not repaired are subject to daily storage charges.  Storage will also be charged on repaired vehicles left in our care in excess of ten days of notification of completion unless prior arrangements are made.

Power of Attorney and Insurance Company Direction to Pay

I do herby appoint Frank's Collision Repair as my attorney-in-fact to accept on my behalf any and all checks, drafts, or electronic funds transfers for deposit to Frank's Collision Repair business account for credit of my account for repair on my vehicle.  I do hereby authorize the paying insurance company to pay Frank's Collision Repair in my name any amounts due for the repairs to my vehicle.
